Cardiac Resynchronization Therapy

چکیده
eart failure is the commonest cardiovascular disease that causes high morbidity and mortality. In a selected group of patients with evidence of electrical dyssynchrony, cardiac resynchronization therapy plus defibrillator backup (CRT-D) was established as an important adjunctive therapy. Numerous landmark trials demonstrated that such therapy improved clinical symptoms and quality of life, reduced heart failure hospitalization and reduced mortality.1 In view of these beneficial results, CRT-D was classified as a Class I indication in both American2 and European3 heart failure device therapy guidelines. However, most of the studies focused mainly on Western populations and it is interesting to know if CRT has a similar beneficial effect in Asian populations.

Cardiac resynchronization therapy for congestive heart failure.
Cardiac resynchronization represents a novel therapeutic strategy for the treatment of congestive heart failure due to systolic dysfunction. Since its modest beginnings in the 1990s, cardiac resynchronization therapy has gained widespread acceptance as a useful adjunct to pharmacologic therapy for congestive heart failure. Randomized trials have consistently shown functional improvement in pati...
متن کامل[Three-dimensional imaging in cardiac resynchronization therapy].
Cardiac resynchronization therapy improves clinical symptoms and prognosis of heart failure patients. However, it has been shown that up to 40% of patients do not respond to this therapy. Three main determinants of cardiac resynchronization therapy response have been identified: left ventricular dyssynchrony, left ventricular lead position, and extent and location of myocardial scar tissue. Two...
متن کاملCardiac Resynchronization Therapy for Heart Failure
The weight of evidence supporting the routine use of cardiac resynchronization therapy, or atrial-synchronized biventricular pacing, as a treatment for patients with moderate-to-severe chronic systolic heart failure and ventricular dyssynchrony is now quite substantial. Results from mechanistic studies, observational evaluations, and randomized, controlled trials have consistently demonstrated ...
متن کاملCardiac Resynchronization Therapy in Women
Despite underrepresentation of women in cardiac resynchronization therapy studies, it has been conclusively shown that women demonstrate greater benefit than men in functional status, reverse cardiac remodeling, and survival and female gender remains an independent predictor of greater response to cardiac resynchronization therapy after adjustment for various other factors. Eligible female pati...
متن کاملCardiac Resynchronization Therapy
Cardiac Resynchronization Therapy. Cardiac resynchronization therapy (CRT) is a recently developed approach to treat dilated heart failure with discoordinate contraction. Such dyssynchrony typically stems from electrical delay that then translates into mechanical delay between the septal and lateral walls. Over the past decade, many studies have examined the pathophysiology of cardiac dyssynchr...
متن کاملEchocardiography in cardiac resynchronization therapy.
Cardiac resynchronization therapy has been an effective option in patients with advanced heart failure. However, 20 to 30% of the patients do not benefit from this therapy. Clinical, electrocardiographic and echocardiographic criteria have been studied in an attempt to select patients who will benefit from a cardiac resynchronization therapy, and the echocardiogram is important both in the sele...
